Arranged by division (Eastern, South Eastern, South Western, Central and Northern) and disease: enteric fever; continued fever or PUO (pyrexia of unknown origin); scarlet fever; erysipelas; cerebro-spinal fever; ophthalmia neonatorum; trachoma; poliomyelitis; polio encephalitis; encephalitis lethargica; pneumonia; influenzal pneumonina; influenza; malaria; dysentery; gastro enteritis; food poisoning; measles; german measles; whooping cough; chicken pox; infective jaundice or hepatitis; mumps; paratyphoid; puerperal pyrexia; diphtheria; Weil disease; typhus fever; smallpox; cholera; leprosy; anthrax; beri-beri; and others (not all categories are populated). Gives date, name of patient, age, address, date of sickening, hospital, date admitted, doctor notifying, school/occupation; milk supply; size of house; number in house and remarks.
