Scope and Content
Envelope contains various notices and leaflets relating to General Practitioner services (including a form to be filled in when committing a patient to an asylum), as well as blank medical certificate forms, headed note paper from the surgery in Braidmead, Irvine, and National Health Insurance order forms. It also contains instructions for patients and general practitioners for a Tuberculin Patch Test kit. A booklet of medical certificates from the Imperial Chemical Industries Ltd Explosives division, who had a base at Ardeer, is also housed within the envelope and does contain some names of employees who had to be signed off work during 1946 and 1947.

The materials within this envelope would have been gathered together by Alexander Percy Walker during his career as a General Practitioner during the latter half of the 1940s, and so does not reflect the original provenance of the records.
